Coaching Checklist
* In order to be considered for an Omaha Hockey Club coaching position, you must complete the below steps.
1.) USA Hockey & MidWest Affilate Forms
Please download, print, complete and return the following forms to the Omaha Hockey Club office by either:
A.) Mailing your completed documents to:

B.) Dropping off your completed documents at the Omaha Hockey Club mail slot at the Moylan Tranquility Ice Plex.
C.) Scanning your completed documents and emailing them to: ggomahahockey@cox.net
2.) Omaha Hockey Club Coaching Application form.
Please take the time to complete all fields and submit.
3.) USA Hockey On-Ice Coach Registration
Upon being selected to a head or an assistant coaching position, you will need to register as an on-ice Player / Coach by clicking here.
The cost will be $38. This registration will also cover you as a player if you still play in addition to coaching. Remember to print out the confirmation page (with barcode) and return it to the Omaha Hockey Club office.
4.) USA Hockey CEP Certification
Your last step to becoming an Omaha Hockey Club coach is to obtain your proper USA Hockey coaching card (certification). Do this by signing up for and completing coaching certification clinics. Coaches have until December 31st of each season to obtain their credentials.
